Please read carefully and answer all questions.

1. Health Profile:
These questions are asked for the purposes of ensuring your safety and permitting us to plan work schedules prior to the start of field school. We typically work outdoors the entire day; it can sometimes get quite hot and uncomfortable, and archaeological fieldwork also can be strenuous.
Information you provide about physical disabilities and medical history is confidential and will not be used as a criterion for acceptance into field school. It is crucial that you are honest with us when answering these questions. State any physical disabilities or anything in your medical history that we should know about (back, knee, or joint problems, allergies, severe poison ivy allergies, heart or respiratory problems, hypoglycemia, diabetes, contact lenses, medications currently being used, etc.) How do you rate your overall physical condition? (select one) Excellent Good Average Fair Poor

2. Academic Work in Anthropology and Other Relevant Disciplines: We require that applicants provide information about their academic history. At a minimum, please provide your current cumulative GPA, along with a description of courses you have taken that you think are relevant to archaeology, and a description of any previous archaeology-related experiences (NOTE: none is necessary, we expect most students to be inexperienced). If you prefer, you may instead paste a copy of your transcript or resume/vita into the form (IU students may highlight, copy, and paste their transcripts from OneStart).

3. Other Skills: Tell us about any other skills you have, such as photography, drafting, carpentry, computer skills, landscaping, first aid, or other non-archaeological work experience, that you think might prove useful on an archaeological excavation.

4. Interests: Briefly state your interests in archaeology (i.e., culture areas, time periods, theory, techniques, etc.). In view of these interests and your experience, what specifically do you expect to gain from participation in the IU-GBL field school program?

Clicking on this check box constitutes my electronic signature attesting that all the information I have supplied in my application is true and correct. I understand that falsifying my application is grounds for refusing me admission to the GBL field school, or for later dismissal.
